I. At presentation âHistory Details
PRIMIGRAVIDA - 8 Months
Symptoms - Cold / Cough / Fever / Dyspnoea/ - 5 days
Vitals-RR - 38cpm Spo2 - 92% with GL of O2
HR - 110bpm
II. Course in Hospital
Admitted with signs of pneumonia and impending respiratory failure was started on antibiotics and supportive care. In view of respiratory distress, patient was initially put on non invasive ventilation[]BIPAP].
In view of pregnancy status, and high suspicious of influenza infection, patient was started on ant flu treatment along with antibiotics and supportive care. Because of increasing intensity of pneumonia and pre morbid gravid status, decision was taken for termination of pregnancy by elective caesarian section, in view of decreasing the stress over the pneumonia status. Patient was put on mechanical ventilatory support and gradually improved in pneumonia status. Over a period of 6 days, patient was gradually weaned off from ventilator support and extubated. Patient symptomatically improved over a period of time and both mother and baby were healthy at the time of discharge.

Total duration of stay in hospital - 16 days
Total duration of stay in ICU - 13 days
Total duration of NIV - 03 days
Total duration of Mech ventilation - 07 days
NO END ORGAN DAMAGE TILL DISCHARGE
IV. Conclusion: Timely intervention by caesarian section and early mechanical ventilator support proved to be effective in saving two lives by reducing the stress over respiratory system.
