My name is hemanta chakraborty. My telephone (broadband) connection no is 2477-4054. I have contacted our harinavi tel exchange for surrender of my broadband connection. But they told me that we have no surrender form please contact ranikuthi exchange. Only for this reason due to some unavoidable circumstances I want to surrender my broadband not telephone connection. So therefore you are requested to please stop my broadband connection with immediate effect. I have already paid Rs.772/- for current tel bill. For comming month I will not pay for broadband bill.
